I hereby consent to participate in the psychoeducation and support group sessions provided by Andrea Redman Consulting.
I understand that these sessions are designed to provide education, support, and guidance and are not a substitute for individual therapy or medical treatment.

I understand that the goals of the psychoeducation and support group include:

Providing education about mental health conditions, symptoms, and coping strategies.
​
Offering a supportive environment to share experiences, concerns, and insights.
​
Learning and practicing skills to manage emotions, relationships, and stress.
​
Building a sense of community and mutual support among group members.
​
I understand that participation in the psychoeducation and support group is voluntary, and I have the right to withdraw from the group at any time without penalty.
I agree that there is no refund given should I decide to voluntary withdraw from the group.

I understand that confidentiality is an essential aspect of the group, and I agree to respect the privacy of other group members by not disclosing any information shared during the sessions outside of the group setting.

I understand that while the facilitator(s) will make every effort to create a safe and supportive environment, they cannot guarantee that all participants will adhere to the group's guidelines or that all discussions will be free from triggers or distressing content.

I understand that my participation in the psychoeducation and support group does not create a therapist-client relationship with the facilitator(s), and they are not responsible for providing individual therapy or crisis intervention services during the group sessions.
​
I have had the opportunity to ask questions about the psychoeducation and support group, and I understand the nature and purpose of the group to the best of my ability.
By signing this consent form, I acknowledge that I have read and understood the information provided and agree to participate in the group sessions voluntarily.
